Hi Jay,
Looking at that page, and the corresponding form, it seems like the issue
here is not with field captions - or field names - but with section
headers. There is some buggy behavior in Semantic Forms when there's a
multiple-instance template specified to go between two sections, and a page
has no instances of that template; I think that's what you're seeing here.
Assuming that's the case, until that bug is fixed I would say your best
option is to change the form definition in one way or another so that you
don't have that pattern showing up.

When I create and save a new form through Semantic Forms, all goes well.
If I "Edit with Form" that page, and don't populate some fields, the field
captions duplicate as well as the field names.
Latest version of Semantic Forms. I'm also using a slightly modified
version of the Bootstrap skin modified for Media Wiki. In order to gain
better control over the styling, I'm not using the traditional captions for
fields, but leaving those empty and using headings instead.
