I switched to ugly URLs and it seems to be working now. I found a couple of
free IIS plugins that do URL rewriting so I'll try and get one of them
working tomorrow. I may also give Apache a whirl, especially if the IIS
plugins don't work well.

Use Apache on Linux/Unix unless you have some very, very good reason not
to. If you must use Windows, use Apache. If you must use IIS, you'll
have to figure it out on your own as we don't have it or support it.
If you can't configure IIS correctly, try switching the $wgArticlePath
to 'ugly URL' style.
