For what its worth, technical discussion about implenting extensions
is in scope for this mailing list (imo), so its perfectly fine if you
ask here, (Of course you should also feel free to email people
directly if you prefer that).

Just to be clear, do you mean MediaWiki or Wikimedia users (Wikimedia
= Wikipedia and related sites. MediaWiki = anyone who uses the
MediaWiki software)?
You can make any sort of MediaWiki extension you want, even if it
duplicates another extension, and people may use it. For Wikimedia,
probably only one extension would be chosen. Wikimedia also generally
has very picky standards for using extensions, and it can be a lot of
work (and politics!) to get an extension used on Wikimedia wikis. The
existing 2 factor auth extension is (AFAIK) only used on wikitech,
which is kind of different from being used on other Wikimedia wikis,
so most wikis have no 2 factor auth solution deployed to them.
I would definitely reccomend coordinating with people working on
similar extensions.

I think you misunderstand the extension status bar on
mw.org. That is
self-reported by the author, and the requirements are not consistently
enforced. The contents of this status really has nothing to do with if
the extension is actually used on Wikimedia wikis, so "taking it out
of beta" would basically mean making a (rather meaningless) edit to
the extension description page.
