Someone not reading instructions. From [[WP:AFC]]:

Also, assuming authorship just because someone created the database row 
in MediaWiki is dubious anyway, since there are merges, copying from 
other sources, etc... Yes, that should always be noted in the edit 
summary. Yes, some people don't. But it's not a /new/ problem.
