I ran into this problem just over a month ago when I became an admin on
en.wiktionary - I'd added my email address but failed to spot the
addtional checkbox.
Perhaps it would be a good idea to make a note of this checkbox's
existence right by where you enter your email address. Is this set on a
MediaWiki: namespace page somewhere or do we need a developer to do it?

I just doublechecked mine, and my address was filled in properly - but
then I tried actually emailing to myself, which didn't work, and then I
noticed the little checkbox at the bottom of the preferences, which I
don't remember ever noticing before. So maybe people have their mail
reception disabled unawares - people have been sending to me via en:, so
I wouldn't have noticed until somebody said something (like now :-) ).
