Looks like we're all thinking along the same lines; I was going to
write a Special:Interwiki page at some point. At least this would
avoid duplication of effort. :-)

Basically, yes, all you need to do is add to / edit the 'interwiki'
table in the database. As it happens, I'm seeing if I can hack
together a Special: page for viewing and editting these right now, but
when/whether it will be included in the distribution, I wouldn't like
to wager.
