I think in the case of place-names, we're pretty lucky in that the
standard US place-name format (City, State) means that there is less
competition for the unadorned article.  This is part of why I fight
vociferously against the idea that comes up every so often that we
should abandon the convention for US placenames.  It avoids a whole
lot of arguments.
