How do you decide you _want_ to see the image before knowing what the
image is about? Someone going to [[Autofellatio]] may have no idea what
it's about, but once they know, may be quite sure they do _not_ want to
see a photograph of it. If the image is unconditionally inlined, you've
removed that choice from them.
That argument runs both ways, I'm afraid.
