I'm not usre I understand that part:

Sadly there are not many hooks in the mediawiki source and on needs to be
added to support this extension. Add the line proceeded by a plus into the
right place (indicated by the line number and lines of content) in your
includes/EditPage.php file.

I need to add wfRunHooks( 'EditPageToolbar', array( &$toolbar, NULL ) ); in
EditPage.php?

Also, what is the name of the default skin?

Been working on it. I was waiting for the guy who started the project to 
respond but in the mean time I will give you the links. My version is a 
proper mediawiki extension but still uses his original javascript. Enjoy.
