This issue has been open since May 2016 with various levels of activity. 

Matanya reported a couple more instances of this yesterday. And Matma
reported another instance back in August. It was made UBN! yesterday as
Matanya put it as a blocker to the wmf.22 train (all blockers are
effectively UBN! by default).

This is not blocking the wmf.22 train[0], but not because it isn't
important. The issue coincides (just correlation!) with wmf.9, so much
further back than we can rollback to.

Action: 
Please take a moment to look into this issue. At this point, this should
either be fixed properly or explicitly stated why it is not a priority.
