with the summary "(recovered file, missing upload log entry)"?
These should appears on the old upload log pages...
Unfortunately all I can see at the moment is these:
http://en.wikipedia.org/wiki/Wikipedia:Upload_log
which only lists back to October 2002.
Phase 2 upload logs would have gone into a page 'Log:Uploads' at the
time, and I haven't been able to track down quite what happened to that.
I've got some old DB dumps from phase 3 days at home, but I don't think
I have anything from phase 2 (that was I think still hosted on a Bomis
server which I never had access to) so I don't know if I can recover all
that.
Additional complications may come from some images having been
auto-copied over from Meta, or being half-uploaded and never fully
recorded correctly.
