This has to do with the non-free nature of the images. The communities have decided they did not want non-free images to be used more than the bare minimum possible, so a non free image like a movie poster is not returned as a page image.

In cases where the opensearch extension doesn't return an image, is there another API which allows extracting the infobox image if one is present?
